Job summary

Interim HealthCare of Northern Illinois is seeking a Home Health Speech-Language Pathologist (SLP) for Libertyville, IL and surrounding areas. The role is PRN with day shifts, offering a competitive hourly rate and mileage reimbursement as you provide home-based therapy.

You’ll work within a small, patient-focused team to deliver personalized care, assess progress, and educate patients and families on therapy goals and safety. This position emphasizes autonomy and flexible assignments.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology; active IL SLP license.
• Minimum 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification.
• Knowledge of state and federal home health regulations.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Collaborate with a home health team to meet patient-centered goals.
• Observe and evaluate a patient’s ability to communicate and swallow.
• Conduct patient assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises; educate patients and families.
• Support safety practices and plan of care adherence.

As a Home Health Speech-Language Pathologist, here’s a big-picture view of what you’ll do:

• Provide home-based speech therapy to patients having communication, swallowing or cognitive difficulties due to an illness or injury
• Work as part of a home health team which may include an RN, LPN, CNA, HHA, PT and OT, focused on the patient’s plan of care and established goals
• Observe and evaluate a patient’s ability to communicate, swallow and process information
• Conduct patient assessments, document progress and ensure patient goals are met
• Assist with mouth, breathing and swallowing exercises, fluency, articulation and reading
• Educate patient and family on plan of care, therapy goals, safety practices and self-care

A few must-haves for Home Health Speech-Language Pathologists:

• Master’s or Doctoral degree in Speech-Language Pathology and active SLP license in Illinois
• Minimum of 1 year of speech therapy experience, ideally in home healthcare
• CPR certification
• Knowledge of state and federal home health regulations
• Good clinical judgement, critical thinking skills, strong communicator, empathy and compassion
