Job summary

Interim HealthCare of Northern Illinois seeks a Home Health Speech-Language Pathologist in Libertyville, IL and surrounding areas to deliver patient-centered therapy in the home. The role focuses on improving communication, swallowing and cognitive function with flexible PRN assignments.

You’ll work with an interprofessional team, assess progress, and educate patients and families on goals and self-care. CPR and Illinois licensure are required.

Qualifications

• Master’s or Doctoral degree in Speech-Language Pathology and active SLP license in Illinois.
• Minimum of 1 year of speech therapy experience, ideally in home healthcare.
• CPR certification and knowledge of home health regulations.

Responsibilities

• Provide home-based speech therapy to patients with communication, swallowing or cognitive difficulties.
• Observe and evaluate a patient’s ability to communicate and process information.
• Conduct assessments, document progress and ensure goals are met.
• Assist with mouth, breathing and swallowing exercises, fluency and articulation.
• Educate patient and family on plan of care, safety and self-care.
