Creekside Transitional Care and Rehabilitation in Meridian, ID is seeking a dedicated Speech-Language Pathologist for our in-house rehab program. This full-time role involves evaluating patients, delivering rehabilitative therapy, and coordinating discharge planning to ensure quality post-acute care.

Responsibilities

• Screen and evaluate patients with communication and related disorders and develop care plans.
• Provide rehabilitative, skilled, and medically necessary treatment interventions.
• Manage physical demands of the role including standing/walking and patient handling.
• Administer procedures as part of the rehabilitation plan (manual techniques, therapeutic exercises, wound care).
• Provide consultation to patients, families, caregivers, and other services.
• Generate comprehensive discharge summaries compliant with regulations.

Creekside Transitional Care and Rehabilitation in Meridian, ID is a post-acute care facility with long-term care, short-term care, and geriatric services provided. We can offer a stable and fun in-house therapy team to work with as well as excellent programs serving our geriatric clientele in their journey to recovery. We are currently seeking a dedicated Full Time Speech-Language Pathologist for our in-house rehab program!

Key Responsibilities

• Conduct effective screening and evaluation of patients with physical functioning disorders and/or wound care, developing appropriate care plans adhering to regulatory and clinical standards.
• Deliver rehabilitative, skilled, and medically necessary treatment interventions to patients with physical functioning disorders and/or wound care, in accordance with regulatory and clinical practice requirements.
• The role involves regular standing and walking throughout the facility and occasional lifting, transferring, and repositioning of patients and equipment. Physical demands vary throughout the day based on patient care needs.
• Administer various procedures as part of the rehabilitation plan, including manual techniques, ambulation, therapeutic exercises, modalities, use of supportive and assistive devices, and wound care.
• Provide consultation and counseling to patients, families, caregivers, and other service providers regarding physical disorders.
• Generate comprehensive discharge summaries of services provided, ensuring compliance with regulatory and clinical requirements.

Qualifications

• Speech-Language Pathologist license is required.
• Open to SLPs at all experience levels. New Grads are welcome!
