Job summary

Best Home Health Providers Inc. is seeking a per diem Speech-Language Pathologist to evaluate patients in their homes across the Bay Area.

You will develop individualized care plans and deliver speech therapy using specialized techniques and devices, collaborating with physicians and the care team. The role requires a Master's/Doctoral degree in Speech-Language Pathology, active CA SLP license, and at least 1 year of clinical experience, plus CPR (AHA) certification and a valid driver's license

Qualifications

• Master's or doctoral degree in Speech-Language Pathology required.
• Active Californian SLP license required.
• Minimum 1 year of clinical SLP experience.
• CPR certification with AHA required.
• Valid driver's license and insured, reliable vehicle.
• Strong communication and organizational skills essential.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.

Education
