Job summary

Best Home Health Providers, Inc. is seeking a Speech Language Pathologist to work directly with patients in their homes across the Bay Area and surrounding communities.

The role focuses on improving communication, function and quality of life through individualized therapy and family education. Per diem position offers flexible scheduling, a rate of $150 per visit, and the potential to convert to full-time based on performance.

Qualifications

• Master's or doctoral degree in Speech-Language Pathology.
• Active Californian SLP license.
• Minimum 1 year of clinical SLP experience.
• Current CPR certification with AHA.
• Valid driver's license and insured, reliable vehicle.
• Strong communication and organizational skills.

Responsibilities

• Evaluate patients and develop individualized care plans to address speech and language deficits.
• Deliver speech therapy using specialized techniques and equipment, and recommend communication devices when needed.
• Partner with referring physicians and the care team to ensure integrated, patient-centered treatment.
• Educate patients, families, and caregivers on treatment plans and goals.
• Participate in case conferences and care coordination meetings.
• Maintain accurate, timely clinical documentation per organization policy.
• Stay current in speech-language pathology practice and theory.
• Perform other duties as required for patient services.
