H2 Health Prime Living in Kentucky is seeking a Part-Time Speech-Language Pathologist to provide 1:1 patient care in senior living communities. The schedule is flexible with 15+ visits per week and a geriatric caseload focused on dysphagia and cognitive/neurologic needs.
You will evaluate and treat patients independently and as part of an interdisciplinary team, ensuring high-quality outcomes and compassionate care across settings.

This position will evaluate and treat a primarily geriatric caseload 1:1 with a mix of dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient’s functional needs.

Education:A graduate of an accredited Speech Language Pathology program.
Experience:Prior experience in outpatient therapy is a plus. Experience with dysphagia is required.
Qualifications:Active CCC-SLP state license.
