Job summary

H2 Health Prime Living in Kentucky is seeking a Part-Time Speech-Language Pathologist to provide 1:1 patient care in senior living communities. The schedule is flexible with 15+ visits per week and a geriatric caseload focused on dysphagia and cognitive/neurologic needs.

You will evaluate and treat patients independently and as part of an interdisciplinary team, ensuring high-quality outcomes and compassionate care across settings.

Qualifications

• Graduate of an accredited Speech Language Pathology program.
• Experience in outpatient therapy is a plus; dysphagia experience is required.
• Active CCC-SLP state license.

Responsibilities

• Provide 1:1 therapy to geriatric patients across a mix of conditions.
• Evaluate and treat dysphagia and cognitive/neurologic impairments.
• Collaborate with interdisciplinary team; work independently as needed.
