Job summary

H2 Health is seeking a Speech-Language Pathologist to provide 1:1 care for geriatric patients in outpatient settings. The role emphasizes dysphagia management, independent judgment, and collaboration with an interdisciplinary team in a flexible full-time schedule.

Prior outpatient therapy experience and a CCC-SLP state license are required. This is a clinician-led organization offering mentorship, professional growth, and a broad network across states.

Qualifications

• Education: Graduate of an accredited Speech Language Pathology program.
• Experience: Prior outpatient therapy is a plus; dysphagia experience is required.

This position will evaluate and treat a primarily geriatric caseload 1:1 with a mix of dysphagia, cognitive impairment, neurologic disorders, and problem-solving programs. The therapist must be able to work independently and in teams and make appropriate decisions based on each patient\'s functional needs.

Requirements

• Education: A graduate of an accredited Speech Language Pathology program
• Experience: Prior experience in outpatient therapy is a plus. Experience with dysphagia is required
