Job summary

H2 Health Prime Living is seeking a Part-Time Speech-Language Pathologist to provide 1:1 patient care in senior living communities. The role involves evaluating and treating a geriatric caseload with emphasis on dysphagia, cognition, and neurologic programs.

Independent work and teamwork are required, with flexible scheduling for 15+ visits per week. We offer national mentorship, in-house continuing education via H2 University, and a strong work-life balance with no weekends or holidays.

Qualifications

• Education: A graduate of an accredited Speech Language Pathology program.
• Experience: Prior outpatient therapy is a plus. Dysphagia experience is required.
• Qualifications: Active CCC-SLP state license.

Responsibilities

• Provide 1:1 geriatric patient care and treatment.
• Work independently and with interdisciplinary teams to determine patient needs.
