Job summary

Appone at Citrus Nursing Center in California is seeking a Speech Language Pathologist to provide comprehensive speech, language, and communication services in post-acute settings. You will screen, evaluate, and treat patients, develop care plans, and ensure quality patient care in collaboration with the interdisciplinary team.

Requirements include a Master of Science in Speech/Language Pathology, state licensure, CPR, and relevant clinical experience; CFY candidates considered.

Qualifications

• Licensure or Certification as required by State guidelines.
• CPR Certification.
• 1 year Speech/Language Pathology experience preferred.
• 1-2 years’ experience in a residential/assisted living or skilled nursing facility preferred.
• CFY (Clinical Fellowship Year) candidates will be considered.

Responsibilities

• Provides, directs and supervises all aspects of quality patient care, including screening, evaluation and treatment, treatment planning, goal setting, family education and documentation.
• Ensures patient care follows organization guidelines, professional standards and state/federal regulations.
• Serves as patient and rehabilitation advocate for internal and external customers with cross-disciplinary coordination.
• Participates in quality assurance and customer service activities.

Provides, directs and supervises all aspects of quality patient care, including, but not limited to; screening, evaluation and treatment, treatment planning, goal setting, family education and documentation, in order to assure the best outcome possible for the patient. The Speech Language Pathologist (SLP) assures that the patient care follows the organization's guidelines; professional standards and community needs, and is in compliance with Federal and State guidelines.

The SLP serves as a patient and rehabilitation advocate for current prospective patients, to all internal and external customers. Ensures cross-disciplinary coordination in order to provide for consistent patient care. Participates in quality assurance and customer service activities.
