Job summary

Phoenix Home Care & Hospice is seeking a Part-Time Speech-Language Pathologist to assess, deliver, supervise and evaluate therapy services for agency patients in Columbia, MO and surrounding areas. The role supports the physician’s Plan of Care and complies with all regulations while providing compassionate home-based care.

Qualifications

• Master’s degree in Speech-language pathology and current state licensure.
• Prefer 1 year of clinical experience in acute care or rehabilitation settings.
• Ability to work in a fast-paced environment with good judgment and flexibility.

Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and per Plan of Care.
• Maintains legible, accurate, timely documentation in accordance with agency standards.
• Communicates and collaborates with the team when updating the patient’s Plan of Care.
• Performs duties in line with agency financial and productivity goals and policies.
• May require travel to patient locations and flexible scheduling.

It provides valuable support to patients with various speech, language, communication, cognitive, and swallowing disorders. Responsibilities

• Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
• Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements.
• Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned.
• Communicates and collaborates with team members when reviewing, revising, and updating the patient’s Plan of Care.
• Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations.
• Performs any other duties as assigned by supervisor.

Requirements

• Education / Licensure: Master’s degree in Speech-language pathology. Current license in the state(s) of practice.
• Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting.
• Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations.
• Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative.
• May require frequent travel and irregular hours.
