Job summary

Phoenix Home Care is seeking a Part-Time Speech-Language Pathologist to provide home-based therapy in Columbia, MO and surrounding areas. The role involves delivering services per physician plans, maintaining compliant documentation, and collaborating with the care team to optimize patient outcomes.

The position requires a Master’s in Speech-Language Pathology with an active state license, and at least one year of clinical experience is preferred.

Qualifications

  • Must have Master’s degree in Speech-language pathology and current license in respective state(s).
  • 1 year of clinical experience preferred, especially in acute care or rehabilitation settings.
  • Ability to work in a fast-paced environment; may involve travel and irregular hours.

Responsibilities

  • Performs direct speech therapy services to patients as ordered by a physician and per Plan of Care.
  • Adheres to agency standards, policies, procedures, and regulatory requirements.
  • Produces legible, timely, and complete documentation of all services provided.
  • Communicates and collaborates with the care team when reviewing or updating the Plan of Care.
  • Keeps agency goals in mind to promote efficient operations and financial accountability.
  • Fulfills other duties as assigned by supervisor.

Responsible for the assessment, delivery, supervision and evaluation of speech therapy services to agency patients in accordance with the established physicians’ Plan of Care and in compliance with the applicable federal, state and agency regulations, policies, and procedures.

Speech-language therapy plays an essential role in the home health model of care. It provides valuable support to patients with various speech, language, communication, cognitive, and swallowing disorders. Whether individuals face difficulty swallowing, challenges with memory and disorientation, or an inability to effectively communicate, speech-language therapy in the home offers significant benefits.

Responsibilities
  • Performs direct speech therapy services to patients as ordered by a physician and in accordance with the Plan of Care.
  • Complies with agency standards, policies, procedures, state and federal regulations, state practice acts for licensed personnel, the Business Code of Ethics, and payer requirements.
  • Provides legible, accurate, timely and complete documentation in accordance with the established agency standards of all care and services provided to patients as assigned.
  • Communicates and collaborates with team members when reviewing, revising, and updating the patient’s Plan of Care.
  • Performs job responsibilities in accordance with agency financial and productivity goals in the promotion of effective and efficient agency operations.
  • Performs any other duties as assigned by supervisor.
Requirements
  • Education / Licensure: Master’s degree in Speech-language pathology. Current license in the state(s) of practice.
  • Experience: Prefer one (1) year clinical experience, preferably in an acute care or rehabilitation setting.
  • Ability to work in a fast-paced, stressful environment, utilizing good judgment, flexibility and patience in all situations.
  • Position requires mental alertness, precision, analytical ability, problem solving, memory, communication, concentration and initiative.
  • May require frequent travel and irregular hours.
