Job summary

Elm Manor Nursing and Rehab Center in Canandaigua, NY is seeking a licensed Speech-Language Pathologist to join our in-house rehabilitation team. You will evaluate, diagnose, and treat speech, language, and swallowing disorders, and develop individualized plans of care in a subacute, skilled nursing setting.

This part-time or PRN role emphasizes collaboration with physicians and nursing staff, documentation, family education, and participation in discharge planning to improve patient outcomes.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current New York SLP license (or eligibility).
• CCC-SLP certification preferred.
• Prior/subacute or skilled nursing setting experience is a plus.
• Strong communication, clinical, and organizational skills.
• Commitment to patient-centered care and outcomes.

Responsibilities

As an integral part of our skilled nursing facility, you will provide evaluation, treatment, and care planning to patients recovering from illness, surgery, or injury. • Part-time or consistent PRN available
