Job summary

Tri Care LLC is seeking a Speech-Language Pathologist (SLP) to join our rehabilitation team in a Skilled Nursing Facility in Bronx, NY. The role offers full-time hours and an hourly pay rate of $55–$63, aiming to improve residents' quality of life through patient-centered care and collaboration with an interdisciplinary team.

You will conduct evaluations, develop treatment plans, document services, and educate residents and families on communication strategies and dysphagia precautions.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current SLP license in the state of New York.
• CCC-SLP preferred; CFY candidates are encouraged to apply.
• Previous experience in a Skilled Nursing Facility or long-term care setting is preferred.
• Strong clinical, organizational, and communication skills.

Responsibilities

• Conduct evaluations and re-evaluations for residents with speech, language, cognitive, communication, and swallowing disorders.
• Develop and implement individualized treatment plans in accordance with resident goals and clinical findings.
• Collaborate with nursing, dietary, and therapy teams to ensure coordinated care delivery.
• Document all services provided in accordance with federal, state, and facility guidelines.
• Educate residents, families, and staff on communication strategies and dysphagia precautions.
