Elm Manor Nursing and Rehab Center in Canandaigua, NY is seeking a licensed Speech-Language Pathologist to join our in-house rehabilitation team. You will evaluate, diagnose, and treat speech, language, and swallowing disorders, and contribute to care planning for patients recovering from illness or surgery.

This role offers part-time or PRN options. The ideal candidate holds a Master’s in SLP, NY license (or eligibility), CCC-SLP preferred, and experience in subacute SNF settings.

Qualifications

  • Master’s degree in Speech-Language Pathology from an accredited program.
  • Current New York SLP license (or eligibility).
  • CCC-SLP certification preferred.
  • Experience in a subacute/skilled nursing setting is a plus.
  • Commitment to patient-centered care and outcomes.

Responsibilities

  • Evaluate and diagnose speech, language, communication, and swallowing disorders.
  • Develop and implement individualized treatment plans.
  • Collaborate with physicians, nursing staff, and rehab team to ensure quality care.
  • Document patient progress and maintain compliance with all regulatory guidelines.
  • Educate patients and families on therapy goals and techniques for carryover.
  • Participate in interdisciplinary team meetings and discharge planning.

As an integral part of our skilled nursing facility, you will provide evaluation, treatment, and care planning to patients recovering from illness, surgery, or injury.   • Part-time or consistent PRN available

Responsibilities:

  • Evaluate and diagnose speech, language, communication, and swallowing disorders
