Job summary

Sigma Health Rehab in Queens, NY is seeking a Full-time Speech Language Pathologist (SLP) to provide skilled therapy in a SNF setting. You will evaluate residents, develop treatment plans, and document progress in accordance with regulatory requirements.

Qualifications

  • Graduate of an accredited SLP program.
  • Current New York State SLP License.
  • Experience in skilled nursing or geriatric care preferred; new grads with relevant experience encouraged to apply.

Responsibilities

  • Administer Speech Therapy evaluations & examinations to diagnose residents & develop treatment plans.
  • Monitor residents' progress and adjust treatments accordingly.
  • Record information on the initial evaluation, treatment, progress, and discharge of clients.
  • Observe and assess progress and prepare detailed reports.
  • Maintain resident confidentiality and ensure compliant handling of information.
  • Ensure compliance with applicable laws, regulations, or standards.
  • Maintain cooperative relationships among healthcare team members.

Key Responsibilities:
