Job summary

Valley Pointe Nursing & Rehabilitation Center in Castro Valley, CA seeks a qualified Speech-Language Pathologist for part-time/on-call duties. You will evaluate patients, develop treatment plans, coordinate with physicians and health-care teams, and document progress and discharge planning.

Active CA SLP license required; degree in SLP; Masters preferred; proficiency with PointClickCare; benefits include health/dental/vision, 401(k), education stipends, and paid time off.

Qualifications

• Bachelor's degree in Speech-Language Pathology.
• Master's or Doctorate in Speech-Language Pathology preferred.
• Active and in good standing California SLP license.
• Proficiency with PointClickCare or similar EMR.

Responsibilities

• Develop treatment plan and obtain approval for services from referring physician.
• Treat patients per the physician treatment plan.
• Communicate with supervisor and other health team members regarding patient progress, problems and plans.
• Record evaluations, daily treatment notes, weekly progress notes, recertification documentation, per Speech-Language Pathologist in the Board State Practice Act and governmental and third party payer requirements.
• Instruct patient's families or nursing staff in maintenance program and caregiver training in preparation for discharge from therapy services.
• Participate in discharge planning.
• Act as a clinical preceptor for affiliating CFY/RFP Speech-Language Pathologist and student programs.
• Comply with the Speech-Language Pathology and Audiology Bureau State Practice Act and the facility policy and procedures.
