Job summary

Angels Care Home Health is seeking a Speech Language Pathologist to provide therapeutic services in patients' homes across Oklahoma. You will assess, diagnose, and develop treatment plans addressing speech, language, swallowing, and cognitive disorders, collaborating with an interdisciplinary team.

Requires a Master’s in SLP, current state licensure, and ASHA CCC. A valid driver’s license and reliable transportation are required; two years of acute care or rehab experience is preferred.

Qualifications

• Master's degree in Speech Language Pathology from an accredited program.
• Current state license in Speech Language Pathology.
• Certificate of Clinical Competence from ASHA (CCC-SLP).
• At least 2 years of experience in acute care or rehabilitation; home health preferred.
• Valid driver’s license and reliable transportation.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain well-being.
• Collaborates with an interdisciplinary team.
