Job summary

Angels Care Home Health in Red Oak, OK seeks a Speech Language Pathologist to deliver in‑home rehabilitation services, assess needs, and collaborate with care teams. You will develop and adjust care plans to restore or maintain communication, swallowing, and cognitive functions for patients in their homes.

The ideal candidate holds a Master’s in Speech Language Pathology, a current state license, ASHA CCC, a valid driver’s license, and at least 2 years of acute care or rehab experience,

Qualifications

• Master’s degree in Speech Language Pathology from an accredited program.
• Current state license to practice Speech Language Pathology.
• ASHA CCC credential required/preferred.
• Valid driver’s license and reliable transportation.
• 2 years’ experience in acute care or rehabilitation; home health preferred.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain well‑being.
• Collaborates with interdisciplinary team to optimize outcomes.
