Job summary

Angels Care Home Health seeks a licensed Speech Language Pathologist to provide therapeutic techniques for patients in their homes. You will plan, implement, and evaluate care plans aimed at restoring or maintaining patient well‑being, collaborating with an interdisciplinary team.

Requirements include a Master’s degree in Speech-Language Pathology, state licensure, CCC-ASHA, a valid driver’s license, and 2 years’ experience in acute care/rehabilitation; home health experience preferred.

Qualifications

• Master’s degree in Speech-Language Pathology from an accredited program.
• Current state license in Speech-Language Pathology.
• CCC from American Speech-Language-Hearing Association.
• Valid driver's license.
• 2 years’ experience in an acute care or rehabilitation setting; home health preferred.

Responsibilities

• Provides coordinated care to patients in their place of residence.
• Plans, implements, and evaluates patient care plans to restore or maintain patient well-being.
• Collaborates with interdisciplinary team.
