Job summary

CONSUMER DIRECTED SERVICES INC. seeks a Speech Language Pathologist to provide one-on-one speech therapy to older adults in private homes and senior living communities. You will evaluate patients, create personalized care plans, and deliver evidence-based treatments to enhance wellness, mobility, and independence.

You will document progress in EMR, collaborate with an interdisciplinary team, and educate patients, caregivers, and staff. Telehealth may be used when appropriate.

Qualifications

• Valid licensure or eligibility in Speech Language Pathology.
• Degree from an accredited program in SLP.
• Proficiency in EMR and basic computer skills.

Job description

Role: Speech Language Pathologist

Responsibilities: Provide one-on-one speech therapy to older adults in private homes and senior living communities, perform evaluations, develop personalized care plans, and deliver evidence-based treatments to support wellness, mobility, and independence. Document patient progress in EMR, collaborate with an interdisciplinary team, educate patients, caregivers, and staff, and incorporate telehealth when appropriate.

Qualifications & Skills: Valid licensure or eligibility in Speech Language Pathology, degree from an accredited program, proficiency in EMR and basic computer skills, current CPR certification, and ability to meet physical demands such as lifting, repositioning patients, and driving.
