CONSUMER DIRECTED SERVICES INC. is seeking a Home Health Speech Language Pathologist to provide evidence-based services to adult patients in their homes. You will assess communication, cognition, and swallowing, and develop individualized treatment plans to promote safe, functional communication and swallowing.

Collaborate with an interdisciplinary team, document in the EHR, and educate families and caregivers to support ongoing progress and safe home-based therapy.
