Job summary

Alpha Home Health and Hospice seeks a licensed Speech Language Pathologist to provide individualized care to older adults in patients’ homes or senior living communities.

You will evaluate, design and deliver evidence-based therapy plans, document care, and collaborate with a multidisciplinary team, educating patients, caregivers and staff on safety and aging in place, with telehealth when appropriate.

Qualifications

• Valid state licensure.
• Accredited degree in Speech-Language Pathology.
• CCC from ASHA is required.
• CPR certification is required.
• Proficiency with computers and EMR systems.

Responsibilities

Role Overview:

This position is for a licensed Speech Language Pathologist providing individualized care to older adults in their homes or senior living communities.

Key Responsibilities:

Conduct evaluations, develop and deliver evidence-based therapy plans, and support patient independence and mobility. Document care accurately and collaborate with a multidisciplinary team. Educate patients, caregivers, and staff on safety and aging in place, incorporating telehealth when appropriate.

Qualifications & Skills:

Valid state licensure, accredited degree, CCC from ASHA, CPR certification, and proficiency with computers and EMR systems. Ability to meet physical demands of in-home care, including lifting, repositioning, and driving.
