Job summary

Innovate Rehab and Wellness is seeking a dedicated Speech-Language Pathologist for a full-time position in a skilled nursing facility. You will assess and treat speech, language, voice, and swallowing disorders, and tailor therapy plans to improve patient outcomes.

Responsibilities include evaluations, treatment planning, patient and caregiver education, and collaboration with the broader healthcare team. Required licensure, Master’s degree, and CPR certification preferred for this role.

Qualifications

• Master’s degree in Speech-Language Pathology (accredited program).
• Valid state SLP license required.
• CCC-SLP from ASHA preferred.
• CPR certification preferred.

Responsibilities

• Assess, diagnose, and treat communication and swallowing disorders.
• Develop individualized treatment plans based on patient needs.
• Provide therapy for articulation, cognitive-communication, and swallowing.
• Educate patients, families, and caregivers on strategies and techniques.
• Collaborate with OTs, PTs, physicians, and nursing staff for comprehensive care.
• Document patient progress and update treatment plans accurately.
