Job summary

Coordinated Therapy Solutions seeks a Speech Language Pathologist to evaluate, diagnose, and treat patients with speech or language disorders and to develop individualized treatment programs.

Responsibilities include direct patient care under physician direction, screening residents, and designing care plans. You will administer tests, educate families, document progress, and tailor therapy tools to patient needs. A valid state license is required.

Qualifications

  • Graduate degree in Speech-language Pathology from an accredited program.
  • State license as a Speech-Language Pathologist is required.
  • Proficiency with applicable state, federal, and professional standards.

Responsibilities

  • Evaluate, diagnose, and treat patients with speech, language, cognitive, or swallowing issues.
  • Under physician direction, provide direct patient treatment per care plan.
  • Screen and evaluate residents and develop appropriate care plans.
  • Plan and conduct rehabilitative treatment programs to restore communication and swallowing abilities.
  • Administer, score, and interpret speech tests.
  • Instruct residents and families to monitor and practice new skills.
  • Complete required forms per policy and regulatory requirements.
  • Maintain accurate patient records.
  • Devise tools and treatments for individual patient needs.

Job Summary:

The Speech Pathologist will evaluate, diagnose, and treat patients with speech or language disorders, formulate individualized treatment programs.

Duties/Responsibilities:

  • Evaluate, diagnose, and treat patients with speech, hearing, cognitive, or swallowing issues.

  • Under a physician’s direction, provides direct patient / resident treatment based on established plan of care.

  • Screens and evaluates Resident and develops appropriate care plan

  • Plans, directs, or conducts rehabilitative treatment programs to restore communicative efficiency of individuals with communication and / or swallowing or cognitive problems.

  • Administers, scores, and interprets specialized hearing and speech tests.

  • Instructs residents, families, and caregivers to monitor speech and provide ways to practice new skills.

  • Completes required forms and documents in accordance with company policy and state and/or federal regulations.

  • Maintains patient records in an accurate and timely manner.

  • Devises special tools and treatments for the specific needs of patients.

Required Skills/Abilities:

  • Knowledge and skills to care for patient needs of the SNF population

  • Must possess problem solving skills to successfully perform job

  • Excellent verbal and written communication skills.

  • Excellent organizational skills and attention to detail.

  • Strong customer service orientation.

  • Must be able to communicate effectively and professionally with other employees, patients, and family members

  • Ability to travel as needed to perform job

Education and Experience:

  • Graduate of an accredited Masters, or Doctorate in Speech language Pathology.

  • State license as a speech pathologist required.

  • Proficiency with all applicable state, federal, and professional standards

Physical Requirements:

  • Prolonged periods of sitting at a desk and working on a computer.
