Qualifications

Responsibilities

• Provide full suite of medication management services for hospital patients.
• Assist with prior authorizations and financial assistance as an off-site team member.
• Educate patients on how hospital pharmacy serves their care.
• Handle all medication needs and ensure adherence is tracked.
• Document patient information in EMR and Shields tools.
