Indiana University Health is seeking a Specialty Pharmacist for a day shift, Monday through Friday, with partial remote work. The role supports patients with oncology, inflammatory, and neurology conditions by clinically reviewing prescriptions and collaborating with clinic teams.

The position involves outreach to patients and providers and participation in the specialty patient management program, with on-call rotations as needed, based at the Plainfield, IN center.

Qualifications

• PharmD or BS in Pharmacy required.
• ASHP-accredited PGY1/PGY2 residency may be required.
• 0-3 years of relevant experience.
• Indiana Pharmacist license or eligibility.
• Board certification or other specialized training may be required.
• BLS certification may be required.
• May require travel to IU Health facilities.
• May require weekend rotations.

Responsabilités

• Clinical review of prescriptions and treatment plans.
• Collaborate with pharmacist and provider teams embedded in clinic.
• Conduct patient and provider outreach as part of the specialty program.
• Participate in the specialty patient management program.
