Qualifications

• Master’s degree required in a related field; relevant experience considered in lieu of a degree.
• Experience in care coordination, case management, or program coordination preferred.
• Licensure: professional license preferred but not required; must register with VA DHP as Resident/Supervisee within 6 months.

Responsibilities

• Coordinate referrals, assessments, and transitions across jail, hospital, residential, and outpatient settings.
• Track participant engagement and treatment needs; support case coordination processes.
• Facilitate communication among treatment providers, partners, participants, and the court.
• Work with RC and TD teams to ensure continuity of care and appropriate services.
