Job summary

AdvantageCare Physicians (ACPNY) seeks a Care Team Supervisor, Specialty (Registered Nurse) in Elmhurst to lead the specialty care team, supervise LPNs and CTA IIs, and ensure high-quality patient care within the Advanced Care Model. The role coordinates daily huddles, staffing, and adherence to nursing standards.

Responsibilities include supervising staff, mentoring, overseeing scheduling, coordinating patient care plans, reviewing charts, and communicating with physicians.

Qualifications

• Graduate of an accredited school of nursing.
• Bachelor’s degree in nursing preferred.
• Active BLS certification.
• ACLS certification required for GI and Cardiology.

Responsibilities

• Provide professional nursing care to patients and support standards for patient satisfaction, safety, and quality.
• Act as liaison between patients, staff and physicians and support the Advanced Care Model.
• Organize clinical supervision to the specialty care team including delegation, mentoring, scheduling and education.
