FountainRx Specialty Pharmacy is seeking a Specialty Care Coordinator to manage and coordinate care for patients on specialty medications. The role requires collaboration with patients, providers, payors, and hubs to ensure access, adherence, and optimal health outcomes.

You will maintain strong communications, handle prior authorizations, and support data-driven improvements while coordinating with various departments to ensure timely medication delivery and patient support.

Formación

• Minimum two years experience in a health-care setting or customer-service is preferred.
• Computer skills that include word processing, and efficient use of the internet and e-mail.
• Excellent communications skills and organizational skills.
• Ability to work independently and as part of a multidisciplinary team.

Responsabilidades

• Serve as the primary point of contact for patients, demonstrating knowledge of the patient care process and coordinating daily operations.
• Maintain high communication standards in verbal, written, and electronic formats with patients and stakeholders.
• Use digital tools to enhance patient communication and monitor data for quality improvement.
• Coordinate with providers to verify records and manage prescription renewals and status communication.
• Assist with payor benefit investigations, prior authorizations, and reimbursement challenges; help patients understand benefits and access financial assistance.
