Job summary

Daymark Recovery Services in Concord, NC seeks a Care Manager to perform assessment, plan development and linkage to services for individuals with behavioral health needs. The role supports high-risk and medically complex clients requiring intensive care coordination and timely intervention.

Under supervision, you will collaborate with providers, family members, and community partners to ensure health, safety, and continuity of care.

Qualifications

• Associates or bachelor’s degree in a human service field with two years MH/SA/DD experience is required.
• RN with two years MH/SA/DD experience is an acceptable alternative.
• Masters with licensure in a human service field adds value.
• Experience in MH/SA/DD and entitlement services is preferred.

Responsibilities

• Provides care management assessment and reassessment, and develops care management plans.
• Refers and links clients to needed services and tracks follow-up.
• Coordinates with interdisciplinary teams to implement plans of care.
• Performs crisis intervention and coordinates with crisis services when needed.
• Monitors progress and maintains documentation for outcomes.

Education

Associates or bachelor’s degree in a human service field with two years MH/SA/DD experience

Licensed RN with two years MH/SA/DD experience

Masters with licensure in a human service field with one year MH/SA/DD experience

Bachelor’s outside of human service field with 4 years’ MH/SA/DD experience
