Job summary

Daymark Recovery Services is seeking a Care Manager at the Richmond Center in Rockingham, NC. This role conducts care management assessment, planning, referral/linkage, and ongoing monitoring to support recovery and health outcomes.

The position supports collaborative treatment planning, crisis intervention when needed, and ongoing education for health promotion. Qualifications include a human services degree and MH/SA/DD experience, with diverse pathway options.

Qualifications

• Bachelor's degree in a human service field with two years MH/SA/DD experience.
• RN with two years MH/SA/DD experience may qualify.
• Masters with licensure in a related field.
• Bachelor's degree outside human services with four years MH/SA/DD experience.

Responsibilities

• Provides care management assessment, reassessment, development of care management plans, referrals and follow-up.
• Participates in interdisciplinary treatment planning and ensures the plan is communicated to all parties.
• Provides crisis intervention when needed to participants of care management.
• All other duties as assigned by supervisor.
