KBR is seeking a Part-Time Special Operations Case Manager to provide nursing case management across clinical settings for DoD beneficiaries in the National Capitol Region. The candidate should have nursing/case management experience, active RN or equivalent licensure, and certifications within a year. U.S. citizenship and security clearance eligibility are required.

Qualifications

• Two years nursing or case management experience; certification within first year.
• Current licensure as RN or LCSW in US territories.
• Ability to use data to develop care plans and analyze outcomes.

Responsibilities

• Collect and organize data for health assessments and care plans.
• Assess patients by telephone using established protocols.
• Oversee discharge planning to ensure timely care.
• Coordinate referrals and negotiate with providers to meet care goals.
• Apply clinical guidelines to ensure standardized, evidence-based care.
