You will apply nursing and case management skills to coordinate high-quality care for DoD beneficiaries. The position demands strong data analysis, discharge planning, and collaboration with providers to optimize patient outcomes while ensuring compliance with regulations and guidelines.

Qualifications

• U.S. Citizenship required.
• Secret or Top Secret clearance (or ability to obtain).
• Two+ years nursing or case management experience; certification in case management within first year.

Responsibilities

• Collect, organize, and communicate health data to develop time sensitive treatment plans.
• Assess patients via telephone using established protocols to provide cost-effective care.
• Prioritize patient care and monitor progress toward goals for coordinated care.
• Oversee discharge-planning to ensure timely, effective post-acute care.
• Assist health care finders to identify patient needs for referrals and services.
• Negotiate and administer service agreements to meet care goals.
• Apply clinical guidelines to ensure evidence-based standardized care for military beneficiaries.
• Understand healthcare finance and resource stewardship to optimize care delivery.
• Use data tools to track and report outcomes to stakeholders.
• Maintain credentialing and ensure compliance with professional standards.
