Job summary

Mercy Housing is seeking a Healthcare Coordinator Specialist to help residents locate and access community health resources, and to coordinate care services to promote self-management and well-being. The role emphasizes collaboration with RSCs and health providers in an on-site setting in Chicago, IL.

Minimum qualifications include a High School Diploma and 2 years of coordination experience; a Bachelor's degree in Healthcare Management or Public Health is preferred.

Qualifications

  • HS diploma or equivalent is required.
  • 2 years experience coordinating with patients, providers, and care teams.

Responsibilities

  • Maintain knowledge of health care providers and services relevant to residents.
  • Coordinate with Resident Service Coordinators to identify residents in need of care coordination.
  • Assess resident assets and connect to resources and personal goal setting.
  • Collaborate with clinics, providers, hospitals, and health plans serving residents at high health risk.
  • Follow consent procedures for information exchange to enable care coordination.
  • Assist residents with health and care coordination services.
  • Collect data and record outcomes for program components.

Healthcare Coordinator Specialist

The Healthcare Coordinator Specialist assists the residents in locating and utilizing community resources to meet their individual health needs. Works with residents to identify their skills and abilities to promote personal growth. Link residents with health care and care coordination services.

This is an on-site position.

PAY

$20-22/hour, depending on experience.

Duties
  • Maintain detailed knowledge of the health care system, key service providers and health care delivery system developments as related to the health needs of residents.
  • Collaborate with Resident Service Coordinators (RSC) to identify residents who are most in need of care coordination and self-management assistance and determine what staff member is most appropriate to provide this assistance.
  • Accurately identify/assess resident assets and needs in order to connect them effectively to resources and help them set personal goals; while recognizing their knowledge and experience.
  • Work with the primary care clinics, medical providers, hospital discharge planning team, care coordinators, health plans and other health service providers who work with the largest numbers of residents at high risk of health problems and elevated health care utilization (high health risk).
  • Follow procedures for consented exchange of information to facilitate collaborative care coordination and effective health self-management by residents.
  • Assist residents with health and care coordination services.
  • Collect required data for designated program components and record data in the Mercy Housing database in an accurate and timely way. Evaluate outcomes of programs and services.
  • Actively collaborate with Resource Development and RS to share information and implement fundraising plans.
Minimum Qualifications
  • High School Diploma or equivalent.
  • Two (2) years of experience in a position that involved coordination among frequent interaction with patience/clients, health care providers, care coordinators, health plans, and other service systems critical to health.
Preferred Qualifications
  • Bachelor’s Degree in Healthcare Management, Public Health, or related field.
