Job summary

FeldCare Connects is seeking a self-motivated Physical Therapist to provide quality home health care to patients of all ages as an independent contractor in Venice and surrounding areas.

You will control your schedule while serving diverse communities, with per-visit compensation based on patient volume. The role emphasizes one-on-one treatment, autonomous practice, and streamlined documentation via Cliniconnects.

Qualifications

• Must hold an active Physical Therapist license in the state.
• License must be current, active, and unrestricted; conditional or provisional licenses are not accepted.
• Successful completion of an accredited Physical Therapy program.
• Reliable personal transportation.
• Smartphone required; access to and use of our app Cliniconnects.

Responsibilities

• Deliver one-on-one home health Physical Therapy to patients in their homes across Venice and surrounding areas.
• Observe, document, and report patient responses to treatment and adjust plans of care accordingly.
• Develop and implement individualized care plans in collaboration with patients and caregivers.
• Maintain accurate documentation using our EMR system during and after visits.
