Qualifications

• Must speak Spanish.
• Knowledge of HHA Exchange.
• 1 year of Intake Experience (Preferred).

Responsibilities

• Receive referrals from referral sources and process referrals from hospitals, nursing homes, and community physicians.
• Verify patient's insurance to determine eligibility.
• Educate patients on services we provide.
• Determine what services were approved by insurance.
• Contact patients to educate them on the services we can provide and assist them with Home Care needs for skilled and non-skilled services.
