Zusammenfassung

Family Service League is a Bay Shore, NY-based social services agency seeking a full-time, Spanish-speaking Case Manager for the Children’s Health Homes program. You will provide care coordination for children and young adults with mental illness and/or chronic medical conditions, conducting meetings to develop strength assessments and individualized planning, and supporting their goal achievements.

Travel is required. Evening hours are required for at least two days a week.

Qualifikationen

• Bachelor’s Degree in Social Work with 2 years of related experience or Master’s Degree with 1 year of related experience required.
• Excellent interpersonal and organizational skills required.
• Strong verbal and written communication skills required.
• Ability to solve problems and work as part of a team required.
• Valid and clean NYS Driver’s License required.

Aufgaben

• Engage with clients who have a primary history of mental illness.
• Develop strengths assessment with clients.
• Offer crisis and safety planning, in collaboration with mental health providers.
• Create individualized planning with clients in health, education, mental health, vocation, socialization, benefits, legal and life skills.
• Meet a minimum of 2 times per month to support youth with goal achievements.
• Coordinate services with providers, benefit systems and the community.
