Job summary

Family Service League is seeking a full-time Case Manager for the Alternatives for Youth Program in Bay Shore, NY. The Case Manager coordinates in-home family support designed to divert involvement in the Suffolk County Probation System with its philosophy founded on empowering parents/guardians and equipping families for success and restoration.

The Case Manager conducts visits in clients’ homes across the county.

Qualifications

• Bachelor's degree, or Associate’s degree with one years of human service experience or High School Diploma with three years of human service experience required.
• At least one year experience working with community resources is required.
• Excellent interpersonal, organizational, and problem-solving skills required.
• Strong communication skills, verbal and written, are required.
• Ability to work as part of a team.
• Valid and clean NYS Driver's license required.

Responsibilities

• The Case Manager will provide linkages, coordination, advocacy and mentoring.
• Engage families in conflict management and skill building.
• Provide direct services, including anger management and parenting skills to youth and parents in order to avoid involvement with the juvenile justice system.
• The Case Manager will conduct assessments and maintain electronic health records of all services provided.
• Conduct home visits throughout Suffolk County.
• All other duties as assigned.
