Champion Care in Madison, WI is seeking a Hospital Liaison to support our skilled nursing and rehabilitation centers across Madison, Beloit, Janesville, and Southern Wisconsin. This role focuses on building hospital partnerships and increasing referrals while coordinating discharge planning with clinical teams.

As a Hospital Liaison, you will engage with case managers, physicians, and community partners to drive census growth, educate referral sources on our services, and advance post-acute care

Qualifikationen

• Experience in hospital liaison or healthcare sales preferred.
• Knowledge of skilled nursing, rehabilitation, and post-acute care.
• Established relationships with hospital and clinical partners is a plus.
• Ability to develop referral sources and census growth.
• Strong communication and relationship-building skills.

Aufgaben

• Develop and maintain relationships with hospitals, case managers, social workers, discharge planners, physicians, and healthcare referral sources
• Generate new skilled nursing and rehabilitation referrals throughout Madison, Beloit, Janesville, and Southern Wisconsin
• Serve as a key clinical and hospital liaison between referral partners and skilled nursing facilities
• Conduct regular hospital visits and build relationships with discharge planning and case management teams
• Educate referral sources on skilled nursing, short-term rehabilitation, nursing care, and post-acute services
• Identify new opportunities for healthcare business development, hospital partnerships, referrals, and census growth
• Maintain consistent communication with hospital case management and discharge planning teams
• Work closely with facility Administrators, Directors of Nursing, Admissions Directors, and regional leadership
• Assist with referral and admissions processes to remove barriers and facilitate timely patient transitions
• Develop relationships with physicians, clinics, healthcare organizations, senior care providers, and other community partners
• Monitor referral activity, healthcare market trends, competitor activity, and business development opportunities
• Represent Champion Care professionally throughout the Madison and Southern Wisconsin healthcare community
