Complete Care is seeking an experienced External Case Manager in the Madison region to serve as a vital link between hospitals, providers, and skilled nursing facilities by developing referral relationships, coordinating admissions, and ensuring seamless transitions of care.

This role focuses on hospital liaison, admissions, and healthcare business development, requiring travel throughout the Madison region and a strong track record of building partnerships to improve patient outcomes.

Qualifikationen

• Bachelor's degree preferred or equivalent healthcare experience.
• Previous experience as Hospital Liaison, Admissions Director, Admissions Coordinator, External Case Manager, or in healthcare business development strongly preferred.
• Skilled nursing, rehabilitation, hospital, or long-term care experience highly desirable.
• Proven ability to build and maintain professional relationships with referral sources.
• Excellent communication, organizational, and time management skills.
• Valid driver's license and willingness to travel throughout the Madison region.

Aufgaben

• Develop and maintain relationships with hospital case managers, discharge planners, physicians, and partners in the Madison region.
• Coordinate referrals and facilitate admissions into Complete Care facilities.
• Partner with Admissions Directors and leadership to ensure timely communication and customer service.
• Conduct hospital and community visits to strengthen referral partnerships and identify opportunities.
• Serve as the primary external liaison during referral and admission processes.
• Collaborate with clinical and operational teams to ensure smooth patient transitions.
• Track referral activity, documentation, and admission trends.
• Represent Complete Care at networking events and community outreach.
