Job summary

Woburn Rehabilitation and Nursing Center is seeking a compassionate Social Worker to support residents and families in a skilled nursing and rehabilitation setting. The role focuses on psychosocial support, care coordination, discharge planning, and collaboration with an interdisciplinary team to promote quality of life.

Responsibilities include psychosocial assessments, counseling, discharge planning leadership, and advocacy for resident rights.

Qualifications

• BSW required; MSW preferred.
• Experience in SNF/long-term care or rehabilitation setting preferred.
• Familiarity with discharge planning and community resources.
• Knowledge of resident rights and healthcare regulations.
• Strong communication, interpersonal, and documentation skills.
• Excellent organizational and time-management abilities.
• Ability to work effectively in an interdisciplinary team.

Responsibilities

• Complete psychosocial assessments and contribute to individualized resident care plans.
• Provide support and counseling to residents and families to assist with adjustment to the facility and rehabilitation process.
• Lead and support discharge planning in coordination with hospitals, families, community resources, and other healthcare providers.
• Collaborate with nursing, therapy, and other members of the interdisciplinary care team.
• Assist with admissions, transfers, and resident and family communication.
• Ensure timely and accurate documentation in accordance with facility policies and applicable state and federal guidelines.
• Participate in resident care plan meetings and follow up on assigned action items.
• Connect residents and families with appropriate community resources and support services.
• Advocate for resident rights, dignity, and quality of life.
