Job summary

Stellar Health Group is seeking a compassionate Social Services professional to support residents and families in a skilled nursing setting. You will assess psychosocial needs, coordinate services, and assist with discharge planning while collaborating with an interdisciplinary team.

The role emphasizes strong communication, documentation, and advocacy for resident-centered care, with an emphasis on navigating healthcare resources and community supports.

Qualifications

• Bachelor's degree in social work or related field; Master's preferred
• LSW licensure required; LCSW/LICSW preferred
• Minimum of two years in a skilled nursing facility
• Experience with short-term rehabilitation and discharge planning
• Excellent communication, documentation, and assessment skills

Responsibilities

• Build positive relationships between residents and staff to support each resident's individual needs
• Identify options and resources that best meet physical, emotional, and psychosocial needs
• Maintain communication with residents' families regarding changes in health, goals, and care planning
• Make appropriate referrals and coordinate services with outside agencies and community resources
• Provide or arrange counseling and supportive services as needed
• Assist residents with financial and legal matters and connect them with resources
• Participate as a member of the interdisciplinary team to support comprehensive resident care
• Review facility policies to help ensure compliance with regulations
• Support residents and families through care and discharge planning
• Advocate for residents' needs, preferences, rights, and quality of life
