Newport Academy is seeking a Residential Services Social Worker to manage a regular caseload at a PrairieCare Residential Treatment Center for ages 10-18. The role focuses on group facilitation, treatment and discharge planning, care coordination, and active collaboration with caretakers and providers.

You will lead weekly treatment team meetings, update master treatment plans, and communicate plans across the care team to ensure patient-centered care.

Qualifications

  • Bachelor's degree in Social Work required; MSW preferred.
  • LSW license required; LICSW or LGSW preferred.
  • One year of work or internship in social work or related field; healthcare setting preferred.
  • Experience with electronic health records and coordinating care with teams.

Responsibilities

  • Facilitate groups and lead adolescent psychoeducation.
  • Develop and revise treatment and discharge plans.

Job description

What You’ll Do

The Residential Services Social Worker carries a regular caseload at the PrairieCare Residential Treatment Center, serving ages 10-18. This position focuses on facilitating groups, treatment planning, discharge planning, care coordination, care conference meetings, communication with caretakers, community providers, and others involved in the care of our patients. Collaborates daily with the multidisciplinary team to advance the plan of care.



  • Uphold and apply Social Work knowledge and understanding of emotional and behavioral health practices and principles.

  • Assess, plan, implement, and evaluate the psychosocial services for patients and share social work expertise with members of the treatment team within the average length of stay of 30-90 days.

  • Facilitate weekly multidisciplinary treatment team meetings and create master treatment plans/update treatment plans.

  • Develop and revise treatment and complex discharge plans and communicate these with patients, caretakers, the treatment team, and direct care staff.

  • Provide psychoeducation to patients, caretakers, the treatment team, and direct care staff regarding diagnosis, symptom. management, recommendations, discharge plans (including leading care conferences and discharge meetings), safety plans, patient rights, financial impact issues, and other pertinent patient care matters.

  • Communicate effectively with external providers, agencies, and government and private payer networks to develop relationships and refer patients to the next appropriate levels of care.

  • Lead adolescent psychoeducation groups.

  • Oriented to ethically provide and advocate for patient-centered, culturally appropriate, age-appropriate care.


What Makes You a Great Fit


  • Bachelor's degree in Social Work required. Master's degree in Social Work preferred.

  • LSW license required, LGSW or LICSW preferred.

  • One year work/internship experience in social work, behavioral health care, or related field required.

  • Experience in healthcare setting is preferred.

  • Experience using electronic health records preferred.

  • Knowledge, experience and understanding of coordinating care within a multidisciplinary team of providers.

  • Experience using Microsoft Office products including Word, Teams, Outlook.

  • Ability to smoothly and rapidly shift between dissimilar tasks.

  • Skills necessary to communicate effectively and efficiently both verbally and in writing.


Compensation for this role ranges from$27-$38.60 per hour depending on licensure and experience.
