Job summary

PrairieCare is seeking a licensed mental health professional to assess, plan, implement, and evaluate psychosocial services for patients across our metro locations. You will provide case management and therapy, conduct suicide risk assessments, lead groups, coordinate discharge planning, and collaborate with the interdisciplinary team to ensure culturally responsive, patient-centered care.

Independent Minnesota licensure or active pursuit is required, along with a Master’s degree in social work

Qualifications

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or related field.
• Independent licensure in Minnesota or actively pursuing licensure as a mental health professional.
• Two years of mental health experience preferred.
• Experience in intensive psychiatric healthcare settings preferred.
• Experience using electronic health records preferred.
• Cultural responsiveness and bilingual language skills strongly preferred.

Responsibilities

• Assess, plan, implement, and evaluate psychosocial services for PrairieCare patients.
• Provide case management and therapeutic services to patients and families.
• Conduct psychosocial and suicide risk assessments.
• Complete discharge planning.
• Lead psychoeducational and therapeutic groups.
• Communicate with patients and families about treatment plan decisions.
• Coordinate care with outpatient providers upon admission and discharge.
• Contribute a clinical perspective to the interdisciplinary team.
• Facilitate group, individual, and family therapy sessions.
• Provide coverage for absent social workers and therapists.
• Communicate with clinical team members and Directors.
• Supervise individuals pursuing clinical licensure when assigned.
• Provide gender-, culturally-, and age-appropriate care.
• Work at different PrairieCare metro locations as needed.

Requirements

• Master’s degree in Social Work, Counseling, Marriage and Family Therapy, or a closely related therapeutic field
• Independent licensure as a mental health professional (e.g., LP, LPCC, LMFT, LICSW) in Minnesota required, or actively pursuing clinical licensure
• Candidates pursuing LPCC with a closely related degree must demonstrate that education meets Minnesota licensure coursework requirements
• Two years of previous mental health experience strongly preferred
• Experience in intensive psychiatric healthcare settings preferred
• Experience using electronic health records preferred
• Cultural responsiveness and bilingual language skills strongly preferred
• Ability to maintain required malpractice insurance
• Proficiency in Microsoft Office, including Outlook and Word
• Effective written and verbal communication skills
• Ability to shift smoothly and rapidly between dissimilar tasks
• Ability to react quickly and calmly in volatile or potentially emergency situations
• Ability to make effective decisions in stressful situationsAbility to manage disruptive and explosive patients after training
