Lifepoint Health is seeking a Social Worker for PRN coverage at the Towanda campus. The role focuses on assessing social, emotional, and environmental needs, planning interventions, and coordinating services to support patients and families.
Responsibilities include interviews, discharge planning, patient referrals, and providing therapy. A bachelor’s degree is required; a master’s is preferred, with state licensure within six months of hire.
Formerly known as Towanda Memorial Hospital, this facility is a satellite campus of Guthrie Robert Packer Hospital, located 17 miles north in Sayre, Pa.
The Towanda campus joined Guthrie in April 2015, serving Bradford and Sullivan counties, offering ease of access to clinical services to those in the surrounding communities. Long-term care services are also provided for patients with a 68-bed skilled nursing unit and a 94-bed personal care home.
The Social Worker. assesses, and plans interventions to help patients cope with social, emotional, economic, and environmental problems.
Evaluate patients' ability to safely perform daily tasks and create personalized care plans tailored to their goals and challenges.

Education: Bachelors required; master's preferred
Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
Certifications: Handle with Care within 30 days of hire.
