Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a PRN Social Worker to join our care team in Paris, KY. The role focuses on evaluating patients and families, coordinating services, and supporting discharge planning to ensure safe transitions.

You will collaborate with a multidisciplinary team to monitor treatment plans, provide therapy, and contribute to community health. This on-site position offers comprehensive benefits and development opportunities.

Qualifications

• Current bachelor's in social work required.
• Licenses: Board Certified Social Worker or related field in Kentucky within 6 months of hire.
• Certifications: Handle With Care within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team members to promote, monitor, and evaluate compliance with patient's treatment plan.
• Assists with discharge planning and processes. Identifies resources, including transportation, housing, healthcare, and social/spiritual services, and provides referrals as part of the discharge plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

What we're looking for

Applicants should have a current bachelor's in social work (required). Additional requirements include:

• Education: Bachelors required; Masters preferred
• Licenses:Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
• Certifications:Handle With Care within 30 days of hire.
