Job summary

Centerpoint Health-Paris, part of Lifepoint Health, is seeking a Social Worker to join our clinical team in Paris, KY. The role focuses on assessment, care coordination and discharge planning to support patient and family needs.

Applicants should hold a BSW with preference for MSW and Kentucky licensure eligibility (CSW/LPCA/LCSW) within six months. We offer comprehensive benefits, professional development and opportunities to impact community health.

Qualifications

• Current Bachelor's in Social Work is required.
• MSW is preferred; licensure in Kentucky within 6 months of hire (CSW/LPCA/LCSW).
• Handle With Care certification within 30 days of hire.

Responsibilities

• Interviews and assesses patients and/or family, caregivers, and legal representatives.
• Determines, prioritizes, provides and/or arranges needed internal and external services.
• Participates in case reviews to evaluate case management and progress.
• Consults with healthcare team to promote, monitor, and evaluate treatment plan compliance.
• Assists with discharge planning and identifies resources and referrals as part of the plan.
• Supervises and/or trains new staff, students, and interns.
• Provides individual, family and group therapy.

How You'll Contribute

• Interviews and assesses patients and/or patient's family, caregivers, and/or legal representatives.
• Determines, prioritizes, provides and/or arranges for needed internal and external services/interventions.

What We're Looking For

Applicants should have a current bachelor's in social work (required). Additional requirements include:

• Education: Bachelors required; Masters preferred
• Licenses: Board Certified Social Worker, or related field (i.e. counseling) in the state of Kentucky (CSW, LPCA, LCSW) within 6 months of hire
• Certifications: Handle With Care within 30 days of hire.
