Job summary

Lifepoint Rehabilitation is seeking a Social Worker Coordinator at UK King's Daughter Medical Center in Ashland, KY. The role focuses on comprehensive social work services within an acute rehabilitation unit, including assessments, treatment planning, and patient education.

The position requires a CSWE-approved master's degree and current licensure, with ACSW desirable. Experience with adult/geriatric populations and healthcare settings is preferred.

Qualifications

• Master's degree from a CSWE approved program and current license/certification to practice.
• ACSW certification is desirable.
• Experience working with adult and geriatric populations; experience with physical disabilities is desirable.

Responsibilities

• Participate in pre-admission screening as requested
• Conduct initial assessments
• Develop individualized treatment plans
• Provide age-appropriate therapeutic interventions
• Evaluate patient and family response to treatment
• Document the course of patient care including progress and ongoing need for treatment
• Establish and maintain therapeutic relationships with patients and families
• Provide patient and family education on tasks and resources that support continued functional independence
• Obtain pre-certification and re-certification of patient insurance coverage as appropriate
• Participate in continuing education and professional development
• Communicate and demonstrate a professional image/attitude for patients, families, coworkers
• Contribute to client relations and marketing activities
• Maintain competency, licenses/certifications, and memberships in applicable professional organizations
• Other duties as assigned

What We're Looking For
