Job summary

Ciena Healthcare's Regency on the Lake-Fort Gratiot facility is seeking a Social Worker to provide psychosocial support to residents and families, ensuring compliance with federal, state, and local regulations while supporting care planning, discharge planning, PASARR compliance, and advanced directive facilitation.

Qualifications

• Bachelor's degree in social work or related field.
• One year supervised social work experience in a health care setting.
• Knowledge of federal and state long-term care regulations.
• Knowledge of chronic disease management and care transitions/discharge planning is preferred.

Responsibilities

• Supervises psychosocial support and services provided by Social Worker and Social Services Designee.
• Carries out supervisory responsibilities in accordance with organizational policies and applicable laws.
• Interviews, recommends hires and trains employees.
• Plans, assigns, and directs workflow.
• Prepares work assignment sheets, considering resident safety and staff qualifications.
• Receives calls and finds replacements for absent staff as needed.
• Transfers employees within units as needed.
• Prepares performance evaluations affecting tenure and wage increases of staff.
• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and ensures proper supervision of those under them.

Education

Bachelor's degree in social work or related field

Active Social Work license in the state (LSW/LMSW/LCSW/LBSW/LLMSW preferred)

Knowledge of federal and state long-term care regulations

• Schedules and adjusts lunch and rest breaks.
• Receives and resolves employee complaints.
• Monitors and corrects job performance of employees.
• Adequately supervises assigned staff and assures that those staff are supervising employees assigned to them.

Education and/or Experience:

• Minimum of bachelor's degree in social work or bachelor’s degree in a human services field including, but not limited to, sociology, gerontology, special education, rehabilitation counseling, and psychology.
• One year supervised social work experience in a health care setting.
• Working knowledge of federal and state long-term care regulations.
• Knowledge of chronic disease management and care transitions/discharge planning is preferred.

Certificates, Licenses, Registrations:

• Current Social Work license in the state as required by state regulations.
• LSW, LMSW, LCSW, LBSW, LLBSW, or LLMSW preferred.
