Job summary

Community Healthcare Network in Harlem is seeking a Social Worker MSW I to assess adolescents’ social and emotional needs and collaborate with clients to strengthen life skills. You will document services in medical records and coordinate referrals within a multidisciplinary team.

Qualifications

• MSW degree and eligibility for licensure where required.
• Experience in adolescent social services is preferred.
• Experience with community health programming is a plus.

Responsibilities

• Assess client’s social and emotional needs and develop a plan with the client.
• Provide or arrange services for client and family; assist with referrals.
• Record relevant data in the client’s medical record.
• Provide short-term goal-oriented social work interventions and track cases.
• Identify resources and establish working relationships with providers.
• Conduct HIV universal counseling with pre- and post-test support.
• Participate in prenatal, family planning and primary care programs.
• Provide health education services to individuals and groups.
• Participate in staff development opportunities and committees.
• Co-facilitate group work for special needs populations.
